29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):      (a)  Jobsite, Front Porch Roof:  On or about May 31, 2016, employees were not protected from falling from this 2 in 12 pitch roof.  The fall hazard to the employees was about 14 feet to the ground.     (b) Jobsite, Main Roof:  On or about May 31, 2016, employees were not protected from falling from this 8 in 12 pitch roof.  The fall hazard to the employees was up to about 25  feet.     (c) Jobsite, Back Porch Roof:  On or about May 31, employees were not protected from falling from this 4 in 12 pitch roof.  The fall hazard to the employees was about 15 feet to the ground.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:   (a) Jobsite:  On or about May 31, 2016, employees were working from a ladder jack scaffold and they were not protected from falling.  The fall hazard was about 11 feet to the front porch roof and about 24 feet to the ground.
