29 CFR 1926.1053(b)(7): Care was not exercised in placing, lashing, or holding a ladder with slip-resistant feet that was used on a slippery surface:    WORKSITE: The Werner, Type II, Model D1224-2 24-foot aluminum extension ladder was placed on top of a large drop cloth that was placed to cover the hardwood floor of the entryway vestibule during the painting process without securing the ladder positively to prevent from displacement.    On or about May 25, 2016, the above ladder was displaced while an employee was performing painting task from the ladder, and the employee sustained serious injuries from the fall approximately 20 feet to the floor.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  WORKSITE: The employees, using ladders to paint the interior walls in the entryway vestibule were not provided with information and training in the recognition of hazards associated with the type of ladder being used or the procedures necessary to eliminate or minimize those hazards.
