29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights.  On 13/5/2016, at the 800 Glenwood Ave, Atlanta, Ga.  job site, employees were permitted to work on scaffolds at heights greater than 10 feet and were not provided full decking on those working levels.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  On 13/5/2016, at 800 Glenwood Ave. Atlanta Ga job site, both Juan Sagredo Sr. and Juan Sagredo Jr. are both train as competent persons and were on site but did not stop work on the scaffolds and/or make corrections to the scaffolds prior to permitting employees work on the scaffolds.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  On 5/13/2016, at 800 Glenwood Ave, Atlanta Ga job site, employees were accessing work levels and were not provided a ladder or other means of access. Employees were climbing the frames of the scaffold as a means of access.  Juan Sagredo was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(e)(1) , which was contained in OSHA inspection number 314209164, citation number 2, item number 3 and was affirmed as a final order on 10/26/2011, with respect to a workplace located at 6001 Cumming Hwy, Ste. 400, Sugar Hill, Georgia, 30518.

29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews.  On 05/13/2016 at 800 Glenwood Ave. Atlanta Ga work site, Owner Juan Segrado and Juan Segrado Jr. were working with employees and were knowledgeable of the work conditions. The employees were working high up with no guardrails for the top level. Nether owner stopped the job to make corrections to the scaffolds.    Juan Sagredo was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(4)(i) , which was contained in OSHA inspection number 314209859, citation number 1, item number 1 and was affirmed as a final order on 10/26/2011, with respect to a workplace located at 4485 Hwy 20 SE, Conyers, Georgia, 30094.  Juan Sagredo was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(4)(i) , which was contained in OSHA inspection number 314209164, citation number 2, item number 4a and was affirmed as a final order on 10/26/2011, with respect to a workplace located at 6001 Cumming Hwy, Ste. 400, Sugar Hill, Georgia, 30518.
