29 CFR 1910.146(c)(8)(i) as required by 1960.8b: When an employer arranged to have employees of another employer (contractor) perform work that involved permit space entry, the host employer did not inform the contractor that the workplace contained permit spaces and that a permit space program meeting the requirement of section was required. Saginaw VA Medical Center (Host employer) did not ensure that Tri Tech Cable Inc. (contractor) had a permit required confined space program that outlined the contractors responsibilities while working a permit required confined space there by exposing contractor employees to potentially hazardous atmospheres (oxygen deficiency), mechanical, and electrical hazards while working in manhole # 98.
