At the establishment located on 2400 Ef Griffin Rd., Bartow, FL, employees performing cleaning of a commercial kitchen area, with the use of a gas powered pressure washing system, were exposed to carbon monoxide at a concentration of 79.6 and 93.2 ppm, time weighted average, on or about, 05/30/2016.

At the establishment located on 2400 Ef Griffin Rd., Bartow, FL, employees performing cleaning of a commercial kitchen area, were exposed to the health and/or physical hazards of chemicals such as, but are not limited to; oven cleaner (composition: potassium carbonate and 2-(2-butoxyethoxy) ethanol, gasoline and carbon monoxide. Exposure includes, but not limited to, skin burns and irritation, seizure, coma, and fatality. No written program was available, on or about, 05/30/2016.
