29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    a- East Side Of Building, 1144 East County Line Road, Lakewood NJ: A company employee was exposed to fall hazards when working within an elevated man lift basket of an articulated boom lift.  This employee was wearing a full body harness but, both ends of the lanyard were attached back to the harness rather than having one end attached to the harness and the other attached to the required tie off point within the man lift basket.   Condition noted on or about 06/02/16.    b - West Side Of Building, 1144 East County Line Road, Lakewood NJ: A company employee was exposed to fall hazards when working within an elevated man lift basket of an articulated boom lift.
