29 CFR 1926.100(a):    Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:   (a) On or about June 2, 2016 at worksite 318 E 46 Th Terr, Kansas City, Missouri.- employees working in an demolition zone were exposed to serious struck by injuries, in that they were not wearing  protective helmets to protect their heads from flying rock and lumber being thrown from the roof.     Abatement certification is not required for  this item.

29 CFR 1926.501(b)(1):   Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   (a)  On or about June 2, 2016, at worksite 318 E 46th Terr, Kansas City, Missouri.-employees were exposed to serious injuries, in that the unprotected sides and edges of the elevated work area was not provided with guardrails or any other means of fall protection. The employees were exposed to an approx. 17-19 ft. fall to the surface below.    Abatement verification is not required for this item.
