29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)  440 CHOCOLATE AVENUE, HERSHEY PA, SOUTH SIDE - Employees accessed the roof of the structure by using a Werner 32 foot extension ladder, and the rails of the ladder did not extend at least 36 inches past the upper landing surface, thereby exposing the employees to fall hazards of approximately 22 feet to the ground below, on or about June 2, 2016.  No abatement certification or documentation required.
