OSHA became aware of this employees injuries during the inspection opened on 6/2/2016.      5/28/2016  Joseph Phelps, the Attendant worker, had two fingers amputated on his right hand after attempting to look out a unguarded large rotating ventilation fan to see if the pressure hoses was working properly.  This employee injury was reported to OSHA on 5/31/2016.
