Location: 238 Mulberry St., Belleville, NJ. a) Employees used a DeWalt Spade Handle Drill for material mixing that was powered by an extension cord that was missing the ground pin. The path to ground was not permanent and continuous; on or about 5/24/16.

Location: 238 Mulberry St., Newark, NJ. a) Employees were working from a tubular welded scaffold that was not fully planked at the working levels. The employees were exposed to a potential fall hazard of up to approximately 20 ft. to the ground below; on or about 5/24/16.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. Location: 238 Mulberry St., Newark, NJ. a) Employees were engaged in stucco operations utilizing a tubular welded scaffold without any means of fall protection. The scaffold did not have a guardrail system and was not fully planked at the working levels. The employees were exposed to a potential fall hazard of up to approximately 20 ft. to the floor below; on or about 5/24/16.
