29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition: a) Maintenance Shop, 48 Midway Drive, West Mifflin, PA 15122: Observed on or about June 14, 2016, the maintenance shop was not kept in a clean and orderly condition. Materials, tools, and other items restricted walkaways and created trip hazards.

29 CFR 1910.24(h): Standard railings were not provided on the open sides of all exposed stairways and stair platforms: a) Basement, middle stairs, 647 4th Street, Braddock, PA 15104: On or about May 18, 2016, the left side (facing down) of the stairway leading into the basement was not equipped with a standard railing. The bottom portion of the stairs (approximately last 10 stairs) was open and there was no top rail or mid rail. Employees using the stairs were exposed to a fall hazard of up to approximately 7 feet to the concrete floor below. b) Basement, middle stairs, 647 4th Street, Braddock, PA 15104: On or about May 18, 2016, the right side (facing down) of the stairway leading into the basement was not equipped with a midrail. The bottom portion of the stairway (approximately last 10 stairs) was open and there was no mid rail. Employees using the stairs were exposed to a fall hazard of up to approximately 7 feet to the concrete floor below. c) Basement, side stairs, 647 4th Street, Braddock, PA 15104: On or about May 18, 2016, the right side (facing down) of the stairway leading into the basement was not equipped with a mid rail. The bottom portion of the stairway was open and there was no mid rail. Employees using the stairs were exposed to a fall hazard of up to approximately 7 feet to the concrete floor below.
