29 CFR 1926.451(e)(6)(i): Integral prefabricated scaffold access frames were not specifically designed or constructed for use as ladder rungs: a) Front of Dollar Tree Store: on 31 May 2016 and at times prior to, employees were exposed to falls of approximately 10 feet from the second scaffold level, in that employees were allowed to access the scaffold using the cross braces of a prefabricated scaffold.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a) Front of Dollar Tree Store: On 31 May 2016 and at times prior to, employees were exposed to falls of approximately 15 feet from the second scaffold level, in that fall protection was not provided while employees were laying brick on the third level of the scaffold. b) Front of Dollar Tree Store: On 31 May 2016 and at times prior to, employees were exposed to falls of approximately 10 feet from the second scaffold level, in that fall protection was not provided while employees were loading brick and mortar on the second level of the scaffold.
