Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.          a. On or about June 2, 2016, at 2412 S. Michigan Ave., in Chicago, IL 60616, employee(s) performing carpentry operations on the roof and second floor were exposed to falls of approximately sixteen (16') feet, created by the presence of inadequately protected and/or unprotected sides.        b.  On or about June 2, 2016, at 2400 S. Michigan Ave., in Chicago, IL 60616, employee(s) performing carpentry operations on all floors of the building were exposed to falls up to thirty five (35') feet, created by the presence of inadequately protected and/or unprotected sides.

Michigan Ave., in Chicago, IL  60616, on the first and second floor, the plywood pieces used to cover the floor holes were not secured as to prevent accidental displacement, thereby exposing employees  to falls of approximately sixteen (16') feet.
