The company's lockout/tagout machine specific procedures did not include specific steps for the removal of locks and restoring the lockout/tagout equipment back into services.  Equipment includes but is not limited to, the Extruders Machines Line 1 - 13, on or about 06/08/16.

The company's lockout/tagout machine specific procedures did not include specific steps for testing the machines or equipment to determine and verify the effectiveness of lockout devices, tagout devices and all the energy source was not identified and/or listed on the extruders machines line 1 - 13, on or  about 06/08/16.

Employees operate the Wilton 20" drill press to punch holes in the metal parts for re-taping, without having the chuck guard in place over the moving part/point of operation, on or about 06/08/16.
