29 CFR 1910.27(f): "Maintenance" All ladders were not maintained in a safe condition. All ladders were n4 ot inspected regularly, with the intervals between inspections being determined by use and exposure: a. 7614 Industrial Highway, Macon, GA (Maintenance Department) - Employees were exposed to fall hazard. Employees were not required to perform inspections of the ladders which are being used frequently for defects and/or missing parts. One of the 6ft. self-supporting ladder was found after an incident, with two missing slip resistance foot pads and the ladder was found to be unstable (wobbly), on or about May 25, 2016.

The company's lockout/tagout machine specific procedures did not include specific steps for the removal of locks and restoring the lockout/tagout equipment back into services.
