29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  a. In the maintenance shop, employees were exposed to a struck by hazard, in that, the Jet drill press model #J2550 and the Grizzly milling machines did not have cup guards when used to drill holes in metal parts, on or about 06/08/2016.    b.  In the maintenance shop, employees were exposed to an amputation hazard, in that, the Jet horizontal band saw SN # N0WJ4964's guards were not adjusted to the material being cut. There was 9 inches of the blade exposed and the adjustable guard was not being used, on or about 06/08/2016.
