In the maintenance shop, employees were exposed to a struck by hazard, in that, the Jet drill press model #J2550 and the Grizzly milling machines did not have cup guards when used to drill holes in metal parts, on or about 06/08/2016.    b.  In the maintenance shop, employees were exposed to an amputation hazard, in that, the Jet horizontal band saw SN # N0WJ4964's guards were not adjusted to the material being cut. There was 9 inches of the blade exposed and the adjustable guard was not being used, on or about 06/08/2016.
