29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  a) Throughout the warehouse - On or about and at times prior to May 25, 2016, the employer did not ensure that employees and the site supervisor who operated stand-up forklifts (Model:  560-OPC30TT, S/N 560-08-A07460) were trained to operate such powered industrial trucks in accordance with the standard, exposing employees to ejection and struck-by hazards.
