29 CFR 1910.106(e)(9)(iii): Combustible waste material and residues in a building or unit operating area were not kept to a minimum, stored in covered metal receptacles and disposed of daily:    (a) On or about June 7, 2016, and at times prior, Superior Lube, Inc. did not ensure that combustible waste material and residues in a building were kept to a minimum, stored in covered metal receptacles and disposed of daily. Employees were exposed to fire hazards in that oily rags were temporarily stored in open 5-gal plastic buckets, then transferred daily to covered metal receptacles which were disposed of weekly.    Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:    (a) On or about June 7, 2016, and at times prior, Superior Lube, Inc. did not ensure that junction boxes were provided with covers  approved for the purpose. Employees were exposed to  contact with electrical hazards in that a junction box located in the East side of the shop, by the Rotella Diesel oil drum, was not covered.     Abatement Note: Abatement certification is not required for this item.

29 CFR 1910.305(c)(4): Snap switches mounted in boxes did not have faceplates installed so as to completely cover the opening and seat against the finished surface.   (a) On or about June 7, 2016, and at times prior, Superior Lube, Inc. did not ensure that switches mounted in boxes had faceplates installed. Employees were exposed to contact with electrical hazards in that a light switch, located in the West side of the shop, was missing a faceplate.    Abatement Note: Abatement certification is not required for this item.
