The employer did not fully document its instruction of each employee at the above addressed job site in the recognition and avoidance of fall hazards and the requirement to utilize the personal fall protection system provided by the employer.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet  above the upper landing surface being accessed:  On or about June 7, 2016 at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
