29 CFR 1910.67(c)(2)(iv): Employees sat and/or climbed on the edge of the basket, and/or used planks, ladders, or other devices for a work position:   a) jobsite at approximately 8399 West Ridge Road in Fairview, PA.  On or about June 8, 2016, a fiberglass extension ladder was used inside the basket of the extensible, articulating boom of the bucket truck.  The ladder was extended 18 feet from the basket to the utility pole to install the new cable on the pole.

29 CFR 1910.268(c)(3): Employers did not provide first aid training, including instruction in artificial respiration, required by this section:   a) jobsite at approximately 8399 West Ridge Road in Fairview, PA.  On or about June 8, 2016, employees installing fiber-optic cable were not given training on First Aid/ CPR.  The employees were working from bucket trucks, around electrical lines, along roadways.  Employees were exposed to fall hazards, electrical hazards and struck-by hazards.
