Employees were exposed to the hazard of being struck by falling tools and/or roof materials.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about June 8, 2016, at the worksite located at 304 East Page Avenue in Malvern, Arkansas, the portable extension ladder used to access and descend from the roof on Building 3 extended approximately 14 inches above the landing surface.  This condition exposed employees to falls from elevations.

This condition exposed employees to falls from elevations.
