29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level: A) Onion boxing Shed area, on or about June 9, 2016, the raised catwalk used by employees to access work platforms did not have a mid-rail as part of the guardrail system, exposing employees to a fall hazard of approximately six feet. B) Onion boxing Shed south east corner, on or about June 9, 2016, the guardrails for the platform going over a conveyor did not have a mid-rail and was not 42 inches high, exposing employees to a fall hazard onto the conveyor.

29 CFR 1910.23(e)(2): Stair rails did not consist of a top rail, intermediate rail and/or posts, with the vertical height not more than 34 inches (86.36 cm) nor less than 30 inches (76.2 cm) from upper surface of top rail to surface of tread in line with: A) Onion boxing shed area, on or about June 9, 2016, the metal fixed stairs used to access the grading catwalk/platform, does not have a mid-rail as part of the stair rails system, exposing employees to a fall hazard of up to approximately six feet. B) Onion boxing shed area, on or about June 9, 2016, the metal fixed stairs used to access the upstairs storage area, did not have a mid-rail as part of the stair rail system, exposing employees to a fall hazard of up to approximately fourteen feet. C) South east corner onion boxing shed area, on or about June 9, 2016, the metal fixed stairs to a cat walk going over a conveyor did not have a stair rail system, exposing employees to a fall hazard of up to approximately five feet.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): A) Feed conveyor for grader #1, on or about June 9, 2016, a 208 volt electrical relay box by a feed conveyor did not have a cover in place over the energized parts, exposing employees to an electrical shock hazard.
