29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  At a commercial construction site, employees performing utility work using a  STIHL TS420 Cutquik Saw on the west side of the building were not provided with eye and face protection exposing the employees to eye and face injuries.
