29 CFR 1910.269(a)(3)(i)(C): Before work begins, the host employer did not inform contract employers the information about the design and operation of the host employer's installation that the contract employer needs to make the assessment required by this section:  (a)  On or about May 24, 2016 in the Switch Yard; Domtar LLC did not provide relevant information on the Allis-Chalmers F-2 cubicle switchgear to include arc flash analysis information, 1-line diagrams, and wiring schematics to the contract employer prior to work commencing.  This exposed employees to electrical shock hazards.

29 CFR 1910.269(l)(1)(iii): Electric lines and equipment were not considered and treated as energized unless they had been deenergized in accordance with paragraph (d) or (m) of this section.  (a)  On or about May 24, 2016 in the Switch Yard; Domtar's employees were instructing the contract employees on work to be performed in close proximity of the lightning arresters.  The lightning arresters were not treated as energized exposing employees to electrical shock hazards.
