Thomas Flick Building Services, Inc., 1109 State Street, Erie, PA  16501, west side of the structure, on or about June 14, 2016 and times prior thereto:  An employee performing ground work activities was walking / working below work activities in the aerial lift man basket which included, but were not limited to, painting tasks.  The employee was exposed to falling items from the man basket while working below, and was not wearing head protection.

Thomas Flick Building Services, Inc., 1109 State Street, Erie, PA  16501, west side of the structure, on or about June 14, 2016 and times prior thereto:  An employee performing ground work activities was walking / working below work activities in the aerial lift man basket which included, but were not limited to, painting tasks. 29 CFR 1910.1200(g)(8):   The employer did not ensure that Safety Data Sheets were readily accessible during each work shift to employees when they were in their work area(s).  A.)  Thomas Flick Building Services, Inc., 1109 State Street, Erie, PA  16501, west side of the structure, on or about June 14, 2016 and times prior thereto:  An employee was utilizing chemicals including, but not limited to, Sherwin Williams 1050QD Siliconized Acrylic Latex Caulk to perform work activities including, but not limited to, filling in nail holes.  The safety data sheet for the chemical was not on-site.
