29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees.  (a) Molding department- On or about June 13, 2016 employees operating the Dino Machine has not received lockout/tagout training.  The employees had been performing lockout/tagout on the Dino machine.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  (a) Molding Department - On or about June 13, 2016 an employee attempting to remove clogged material at the Dino machine waste splitter was not guarded resulting in the amputation of employees left thumb that was reattached in surgery.

29 CFR 1910.335(a)(2)(ii): When normally enclosed live parts were exposed for maintenance or repair, they were not guarded to protect unqualified persons from contact with the live parts:  (a) Molding Department - On or about June 13, 2016 the operator of the Dino Machine was reaching in a high voltage cabinet to de-energize the Dino machine.  Operator was exposed to live wires.
