29 CFR 1910.178(a)(5): If the truck is equipped with front-end attachments other than factory installed attachments, the user shall request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered.  a. At the workplace, on or about 06/14/16, employees operated a Hyster H50 forklift that had an aftermarket clamping attachment used for transporting slabs.  The forklift's did not have a capacity plate to reflect the forklift and attachment combination.   b. At the workplace, on or about 06/14/16, employees operated a Mitsubishi FGC25N that had an aftermarket boom and clevis attachment used for material hosting.   The forklift did not have a capacity plate to reflect the forklift and attachment combination.
