29 CFR 1926.251(a)(6): Each day before being used, and during use, where service conditions warrant, the sling and all fastenings and attachments were not inspected for damage or defects by a competent person, and damaged or defective slings were not immediately removed from service: Bridge span #4 and bridge cap#5: On or about June 16, 2016 and at times prior thereto, employees were exposed to a struck by hazard while using a defective yellow nylon sling for lifting bundles of blue Styrofoam onto the bridge and bridge cap.

29 CFR 1926.502(d)(8): Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two: On bridge span#4 and bent cap#5: On or about June 15, 2016, and at times prior thereto; employees were exposed to a fall hazard while connecting to a transverse lifeline that was not designed to be used as part of the horizontal lifeline system in accordance with the original qualified design.
