29 CFR 1910.178(q)(6): Industrial trucks shall not be altered either by the addition of extra parts not provided by the manufacturer or by the elimination of any parts, except as provided in paragraph (q)(12) of this section.   (a) In the fabrication area, the original forks on the Clark forklift SN 91582725 had been replaced with 10ft long forks exposing employees to a struck by hazard.  .

29 CFR 1910.254(c)(1): Installation of arc welding equipment including power supply was not in accordance with the requirements of Subpart S:   (a) In the welding area, the welders operating equipment such as, but not limited to, the Millermatic 350 P MIG were exposed to an electrical hazard when plugging/unplugging the 45 amp electrical plug, in that, strain relief was not being used.
