29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures.  Location: 633 Mt, Prospect Ave, Newark, NJ 07104  a.) Employees engaged in residential construction entered and egressed the construction site while having to walk near a breaker box with unprotected live parts 4ft. from the ground. The breaker box and its live parts were not provided a cover for employee protection; there-by exposing employees to electrocution or electric shock hazards.   CSHO observed violation on or about 6/03/2016.

29 CFR 1926.405(j)(1)(i): Fixtures, lampholders, lamps, rosettes, or receptacles had live parts normally exposed to employee contact.  Location: 633 Mt, Prospect Ave, Newark, NJ 07104  a.) Employees engaged in residential construction were utilizing receptacles that did not have a cover over the fixture to protect employees from coming in contact with live wires and parts; there-by exposing employees to electrocution or electric shock hazards. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Location: 633 Mt, Prospect Ave, Newark, NJ 07104  a.) Employees were engaged in construction activities on a residential roof approximately 25 ft. to the ground below without utilizing any means of fall protection; there-by exposing employees to fall hazards approximately 25 feet to the ground below.     CSHO observed violation on or about 6/03/2016.

29 CFR 1926.1053(b)(4): Ladders shall be used only for the purpose for which they were designed.  Location: 633 Mt, Prospect Ave, Newark, NJ 07104  a.) Employees utilized a folded "A" frame ladder that was leaning on a residential house in order to access the upper roof level; there-by annulling the designed purpose and exposing employees to fall hazards approximately 15ft to the next lower level.     CSHO observed violation on or about 6/03/2016.
