29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a. 225 Bay Street Jersey City, NJ   Employee(s) were engaged in masonry activities from a pump jack scaffold at heights of approximately 20 feet above the ground level. The employer did not ensure the use of fall protection, thereby exposing employee(s) to fall hazards.  Violation occurred on or about 06/20/2016

Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews.  a. 225 Bay Street Jersey City, NJ   Guardrail systems were not installed along all open sides and ends of platforms thereby exposing worker engaged in masonry activities to an approximate 20 ft fall hazard.
