29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:     (a) Jobsite, 300 West Campus Avenue, Davidsville, PA:  On or about 06/21/2016 a flexible cord used to supply power to a Bosch hammer drill was missing the ground pin thereby not providing a permanent and continuous path to ground.

29 CFR 1926.405(g)(2)(iii): Flexible cords were not used in continuous lengths without splice or tap:  (a) Jobsite, 300 West Campus Avenue, Davidsville, PA:  On or about 06/21/2016 a flexible cord used to supply power to a Bosch hammer drill had numerous cuts and slices in the insulation and was not continuous in length.
