Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    345 N. Canal Street, Chicago, IL. - On or about June 16, 2016, employees were required to install electrical conduit, a 440V transformer, and run a power line for a 200 amp circuit breaker. During the installation employees were allowed to work near an exposed energized electrical panel box (PP-1). The employer did not ensure that employees used protective equipment, including electrical personal protective equipment such as, but not limited to, an arc-rated face shield, nonconductive head protection, and flame resistant clothing during live electrical work. Employees were thereby exposed to the hazards associated with live electrical parts and arc flash.
