29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about June 22, 2016 on the roof of the garage, two employers working at installing roofing sheathing and shingles had not been trained to recognize the hazards associated with fall hazards.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.   On or about June 22, 2016 at the garage, three employees were installing roofing material used a 20 foot extension ladder to access/egress the working level, use a ladder did not extend at least 3 feet above the landing.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about June 22, 2016, at the garage, two employees installing roofing and sheathing material were exposed to a fall hazards of approximately twelve feet to the lower level, when the fall hazard included, but were not limited to:  a. one employee working at the edge of the roof was exposed to a fall to the lower when he was not attached to the rope that was part of his fall protection.  b.  one employee working on the roof installing roofing shingles was attached to a 50 foot long rope, but the rope the employees was attached to was too long and the employee would strike the ground if he fell.  The (employer name) was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1) which was contained in OSHA inspection number 1112763, citation number 1, item number 1 and was affirmed as a final order on (date), with respect to a workplace located at 32218 Tobie Circle in Scottdale, GA.
