29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a. At the worksite, on or about 7/28/16: Employees performing roofing work on a single family, residential home. Portions of the roof were approximately twenty-two feet from eve to ground. The employees were exposed to a fall hazard in that guardrail systems, safety net systems, or personal fall arrest systems were not used.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a. At 219 Delores Terrace South, Syracuse, NY 13212, on or about 6/28/16: Employees accessing a residential roof approximately twenty-two feet above the ground. A wooden extension ladder was used to access the roof. The side rails of the ladder extended two feet beyond the landing surface.
