In the stainless steel shop, the employer, in the process of training an employee on the operation of a press brake machine, did not ensure that the operator was supervised continuously and was using the proper tools which exposed the employee to amputation hazards.

In the welding shop area, an unused compressed gas cylinder was stored without the protective cap exposing employees to struck-by hazards.
