29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:     New gym construction, southeast: On or about June 29, 2016, and at times prior thereto, employees were exposed to a fall hazard greater than 6 feet without fall protection, while erecting metal beams and removing cement forms from the concrete slab.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:     New gym construction, southeast: On or about June 29, 2016, and at times prior thereto, employees were exposed to a fall hazard while accessing the concrete slab to conduct steel erection and concrete work from a stairway with more than four risers that did not have any handrails.
