At the work place located at 8028 Newark Road, Mount Vernon, Ohio, on or about June 29, 2016 and at times prior, employees were performing maintenance, configuration changes, and adjustments on two MBO Binders. an MBO Folder, two Polar 78 Cutters, and a Kimori Lithrone 528 2026 multi-color printing press without the use of lockout/tagout.  Employees did not de-energize, apply lockout devices, and verify that the hazardous energy sources were isolated prior to performing the work.  Hazardous energy sources included electric, pneumatic, hydraulic and gravity.

Kimori printing press L-528 had unguarded ingoing nip points in the yellow ink module, exposing employees to a caught-in hazard.    b.  At the work site located at 8028 Newark Road, Mount Vernon, Ohio, employees used a Kimori L-528, Number 2026 multicolor printing press to print brochures, flyers, posters, advertisements and inserts.  Kimori printing press L-528 had unguarded ingoing nip points in the red ink module, exposing employees to a caught-in hazard.     c.  At the work site located at 8028 Newark Road, Mount Vernon, Ohio, employees used a Kimori L-528, Number 2026 multicolor printing press to print brochures, flyers, posters, advertisements and inserts.  Kimori printing press L-528 had unguarded ingoing nip points in the blue ink module, exposing employees to a caught-in hazard.    d.  At the work site located at 8028 Newark Road, Mount Vernon, Ohio, employees used a Kimori L-528, Number 2026 multicolor printing press to print brochures, flyers, posters, advertisements and inserts.  Kimori printing press L-528 had unguarded ingoing nip points in the black ink module, exposing employees to a caught-in hazard.    e.  At the work site located at 8028 Newark Road, Mount Vernon, Ohio, employees used a Kimori L-528, Number 2026 multicolor printing press to print brochures, flyers, posters, advertisements and inserts.  Kimori printing press L-528 had unguarded ingoing nip points in the brown ink module, exposing employees to a caught-in hazard.    f.  At the work site located at 8028 Newark Road, Mount Vernon, Ohio, employees used a Ryobi 3302H mail processing unit to process large batches of mass mailings.  Ryobi 3302H had unguarded ingoing nip points between the printing and processing modules, exposing employees to a caught-in hazard.    g.  At the work site located at 8028 Newark Road, Mount Vernon, Ohio, employees used a Paragon 1947-0000 SN 0208 0005 ink-jetting stream feeder to place and print labels on materials for mailing. The Paragon stream feeder had unguarded ingoing nip points in the processing unit, exposing employees to a caught-in hazard.
