29 CFR 1910.24(f): All treads were not reasonably slip-resistant and the nosings were not of nonslip finish: (a) Facility, 209 21st Street, Pittsburgh, PA: On or about 06/29/2016 the treads of the stairs between the basement and work floor were not reasonably slip-resistant. Employees travel between the two levels several times a day to get supplies and materials.

29 CFR 1910.24(g): "Stairway platforms." Stairway platforms shall be no less than the width of a stairway and a minimum of 30 inches in length measured in the direction of travel: (a) Facility, 209 21st Street, Pittsburgh, PA: On or about 06/29/2016 there was no platform that led in the direction of travel to the stairway. The access to the stairs is perpendicular to the stairway and employees have to turn 90 degrees and step down onto the first step to descend and then turn and step up when ascending.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: (a) Facility, 209 21st Street, Pittsburgh, PA: On or about 06/29/2016 the employer did not develop, implement or maintain a written hazard communication program that addresses product labeling, maintenance of safety data sheets and employee training. Employees work with products such as but not limited to: degreasers, bleach and window cleaners. The employer does not have a program to address labeling of the products; did not maintain the safety data sheets for each product; and did not conduct training for employees who work with the products to ensure they understood the potential hazards associated with each product used.
