On or about February 18, 2016, and in times prior thereto, unapproved jobmade trash boxes were elevated on the forks of a Genie telehandler.  Genie does not approve of the use of these boxes in that they have not been professionally designed, reviewed and load charts have not been created for them.

29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks.  Reference from ANSI B56.1 -169, Safety Standards for Powered Industrial Trucks. Part III Section 6 Operator Safety Rules and Practices, 605 Loading, Paragraph A:  Handle only stable or safely arranged loads.  On or about February 18, 2016, and in times prior thereto, job made wood boxes were used on the front of a Genie telehandler for material handling. Such boxes were not constructed and used to ensure secure stable loading, exposing employees to the hazards of being struck by materials and falls when entering the box.
