29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Zortman Construction, 2120 Colonial Rd. Harrisburg, PA - On or about July 1, 2016, two employees working on a 6:12 pitch roof engaged in residential construction were not protected against fall hazards, nor were they wearing personal fall arrest system equipment. The height of the roof eaves was approximately 20 foot at the second story and 30ft at the peak of the building.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.502(a)(2): The employer did not provide and install all fall protection systems required by this subpart for an employee, and did not comply with all other pertinent requirements of this subpart before that employee began the work that necessitates the fall protection:  (a) Gray Stone Public House, 2120 Colonial Rd. Harrisburg, PA - On or about July 1, 2016, the employer did not ensure that employees working on a 6:12 pitch roof with a 20 second story eaves height and 30 foot roof peak height were wearing the fall protection equipment available to them at the site.     Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  Zortman Construction - On or before July 1, 2016, the employer had not provided safety training to employees to recognize the hazards of falling from a metal roof or of the procedures to follow to protect themselves. The roof measured approximately 30 foot to the peak.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
