29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit.":     a) In the warehouse, on or about July 5, 2016, and at times prior to, the second exit door in the warehouse was not marked by an EXIT sign, exposing employees to entrapment, burn and smoke inhalation in case of fire emergency.  Employees work 8 hours per day in the warehouse, load and unload trucks, sew clothes, price and tag the clothes and do shipping and receiving.

29 CFR 1910.178(l)(6): The employer did not certify that each operator had been trained and evaluated as required by paragraph (l) through a certification that included the name of the operator, the date of the training, that date of evaluation, and the identity of the person(s) performing the training or evaluation:  a) In the warehouse, on or about July 5, 2016, and at times prior to, an employee operates a forklift to unload and load trucks and put merchandise on the shelves.  The employee was not certified by his employer though stated to have been trained and certified by his previous employer Randstad two years ago.  The employer was not able to provide documentation.
