29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated    a)  At the jobsite, on or about 6/21/16:  The employer did not ensure that employees using a JLG aerial lift to gain access to a roof peak, and who elevated that basket to a height of approximately 35 feet from ground, did so with while attached to the boom or basket.     Abatement documentation must be submitted for this item.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a)  At the jobsite, on or about 6/21/16:  The employer did not ensure that employees working on a 14:12 pitch roof, approximately 35 feet from ground, did so with fall protection.     b)  At the jobsite, on or about 6/21/16:  The employer did not ensure that employees working on a 4:12 pitch roof, with a ground to eave height of 10 feet, did so with fall protection.        Abatement documentation must be submitted for this item.
