On or about July 6, 2016, during the replacement of a residential sanitary sewer line at 5779 Colonial Blvd, Willoughby, Ohio, the employer did not ensure employees working inside a trench were protected from excavated material that posed a hazard of falling or rolling into an excavation, by placing and keeping the material at least 2 feet from the edge of a trench or by the use of a retaining device.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c).      On or about July 6, 2016, during the replacement of a residential sanitary sewer line at 5779 Colonial Blvd, Willoughby, Ohio, the employer did not ensure employees working inside a trench, with varying depth of approximately 8 feet to approximately 9.5 feet deep and near 90 degree vertical walls, were protected from cave-ins by an adequately designed protection system.  The soil was classified as; Cohesive Sandy Clay, type B.
