29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   (a). Worksite located at 2305 Summit St., Kansas City, Missouri- Employees were observed working on a steep roof and were not protected from falling by the use of any means of fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a). Worksite located at 2305 Summit St., Kansas City, Missouri- Employees exposed to fall hazards while working on a steep roof had not been trained on the use and application of  fall protection measures to be taken while on the roof.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:   (a). Worksite located at 2305 Summit St., Kansas City, Missouri- Employees were observed using a ladder to access the upper landing surface that was not extended at least 3 feet above the landing surface.
