29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    a) Porch roof, Residence, 80 East Carey Street, Plains, PA - The east-west laying fabricated scaffold plank of a ladder jack scaffold placed on the porch roof of a residential establishment lacked fall protection exposing employees to a fall hazard of eighteen-foot (18') above an asphalt driveway, as observed on or about July 7, 2016.    b) Ground/porch roof, Residence, 80 East Carey Street, Plains - The north-south laying fabricated scaffold plank of a ladder jack scaffold placed on the ground and porch roof of a residential establishment lacked fall protection exposing employees to a fall hazard of eighteen-foot (18') above ground level, as observed on or about July 7, 2016.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a) Main roof, Residential Establishment, 80 East Carey Street, Plains, PA - Employees on the low sloped main roof of a residential establishment engaged in replacing the roof were not protected from falling and were exposed to a fall hazard that ranged from twenty-foot (20') to twenty six-foot (26') above an asphalt driveway, as observed on or about July 7, 2016.
