29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a.   On or about July 7, 2016, on the south side of the new residential construction house, two employees installing framing to the structure while using a pneumatic nail guns did not wear eye protection, exposing the employees to the hazard of being struck in the eye or face by flying debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  a.  On or about July 7, 2016, on the south side of a new construction residential house, three employees installing framing were not protected from an 23 feet 5 inch fall hazard by the use of a personal fall protection system.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  a.   On or about July 7, 2015, on the south side of a new construction residential house, an employee installing framing materials was straddling a folding ladder by standing on the step side and the rear horizontal side while working on the third level porch, exposing the employee to a 23 feet 5 inch fall hazard.
