29 CFR 1926.451(e)(1): Where scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers or stairway-type ladders was not used for access:    a) Jobsite - The employees were working off of a ladder jack scaffold platform, approximately 17 feet above the point of access and did not have proper access for the employees to reach the working level of the scaffold. This hazard exposed the employees to a fall hazard.     Condition was observed on or about June 29, 2016.

29 CFR 1926.451(g)(1)(i): Each employee on a ladder jack scaffold was not protected by a personal fall arrest system:    a) Jobsite - The employees were working from a ladder jack scaffold, approximately 15 feet long and 17 feet high, without the use of a fall protection system. Employees were exposed to a fall hazard of approximately 17 feet. Condition observed on or about June 29, 2016.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    a) Jobsite: A written Hazard Communication Program was not developed and implemented where employees are exposed to chemicals, including but not limited to limestone, gypsum, quartz, etc. contained in Portland cement used during masonry activities.      Condition noted on/about June 29, 2016
