29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    a) In front of the house, on or about July 8, 2016, an employee was exposed to an 18-foot fall hazard while performing work from the 3rd level of a tubular welded frame scaffold that was not fully planked.

29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph:    a) In front of the house, on or about July 8, 2016, two employees used a 4 tier scaffold, 24 feet in height and 5 feet in width, that was not restrained from tipping over, exposing employees up to an 18-foot fall hazard.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    a) In front of the house, on or about July 08, 2016, an employee was working from a tubular welded frame scaffold without guardrails and no other means of fall protection, exposed to an 18-foot fall hazard.
