29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:  On or about July 10, 2016, the employer failed to ensure the working platform was a least 18 inches wide. The employee was working from scaffolding that was 12 inches wide. Employee exposed to a fall hazard of approximately eight feet and five inches.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:  On or about July 10, 2016, there was no safe access to ascend or descend the scaffolding. The employee was exposed to a fall hazard.
