29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) Location: Jobsite Two employees were seriously injured on a jobsite which had not been adequately inspected to ensure the workplace was free from recognizable hazards, when the extensible boom aerial lift that they were working from overloaded the floor and crashed through to the basement level.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: Location: Jobsite Two workers were seriously injured while working from an extensible boom aerial lift that crashed through a floor. The workers were not wearing hard hats with one employee sustaining serious head trauma.
